Launched on 8 September 2014, the Horowhenua/Ōtaki Children’s Team – Tamariki te Tuatahi - has been making their region a better place for children where they can grow up safe, healthy and connected.

As a result of the Children’s Team’s hard work we are seeing direct positive outcomes for vulnerable children in the Horowhenua/Ōtaki community.

Refer a child

If you are concerned about a child (unborn to 18) you can refer them to the Children’s Team. Referrals to the Children’s Team can be made by agencies, practitioners (e.g. teachers and nurses), friends or the families themselves. We do not accept anonymous referrals and the family must verbally consent to the referral being completed. Read more about referring to Children's Teams.

To refer a child to the Horowhenua/Ōtaki Children’s Team:

  1. download and complete the referral form [DOCX, 295 KB]
  2. download and complete the consent form, with the child's family [DOCX, 111 KB]

Then drop the completed form off to our office at 14 Durham Street Levin. Alternatively, you can also email or call (06) 949 8793 for a member of our team to come and collect it. The forms should be in a sealed envelope, addressed to the Children’s Team and marked ‘Private and Confidential’.

Please do not email the referral form with any personal information about the child or family/whānau.

Jacqui Moynihan, Horowhenua/Otaki Children's Team Director

Jacqui Moynihan has been involved in the Horowhenua/Ōtaki Children’s Team since 2014 when the team was established.

Jacqui Moynihan sees her role leading the Children’s Team in Horowhenua/Ōtaki as the culmination of years of working to help vulnerable children to thrive, achieve and belong. Jacqui is on secondment from the New Zealand Police.

Working alongside the Director in Horowhenua/Ōtaki are Children’s Team Coordinator, Alice Sooalo and Team Administrator, Jess Ward.

Horowhenua/Ōtaki Local Governance Group

The Horowhenua/Ōtaki Local Governance Group (LGG) is comprised of key representatives from the local community, government agencies, non-government organisations and iwi.  This group of community leaders ensures the Children’s Team Director has the resources necessary to address the needs of the vulnerable children and their family/whānau across the local community.

Horowhenua/Ōtaki Children’s Team panel

The Horowhenua/Ōtaki Children’s Team panel is made up of a range of sector experts and practitioners who are responsible for reviewing referrals, assigning a Lead Professional to work with the child and their family, and approving and monitoring the progress against the Child’s Plan.
